What Is Gum Disease — and Why Does It Matter?
Gum disease — also called periodontal disease — is an infection of the tissues that hold your teeth in place. It begins when bacterial plaque accumulates along and below the gum line, triggering inflammation. Left untreated, this inflammation progressively destroys the gum tissue and underlying bone that support your teeth.
Gum disease is the leading cause of tooth loss in adults worldwide. It is also one of the most under-diagnosed conditions in dentistry — because in its early stages, it is usually painless. Bleeding gums, bad breath and receding gums are common early warnings that most people dismiss or attribute to brushing too hard.
Beyond tooth loss, research has established clear links between periodontal disease and systemic health conditions including cardiovascular disease, type 2 diabetes, stroke and adverse pregnancy outcomes. Treating gum disease is not just dental care — it is part of managing your long-term general health.
The Stages of Gum Disease — From Gingivitis to Periodontitis
| Stage | Condition | What Is Happening | Symptoms | Reversible? |
|---|---|---|---|---|
| Stage 1 | Gingivitis | Plaque at gum line triggers inflammation. No bone involvement yet. | Bleeding on brushing, red or puffy gums, mild bad breath | Yes — fully reversible with treatment and improved home care |
| Stage 2 | Mild Periodontitis | Infection spreads below gum line. Early bone loss begins. Pockets forming between teeth and gums. | Bleeding, gum recession beginning, possible sensitivity | Arrested and stabilised — not reversed |
| Stage 3 | Moderate Periodontitis | Progressive bone loss. Pockets deepening. Tooth stability begins to be affected. | Gum recession, loose feeling in teeth, persistent bad breath | Arrested with treatment — regular maintenance essential |
| Stage 4 | Severe Periodontitis | Significant bone loss. Teeth visibly mobile. High risk of tooth loss without intervention. | Tooth movement, pain on biting, visible long teeth due to recession | Surgical intervention to arrest progression — some teeth may be lost |
Signs and Symptoms of Gum Disease — When to Seek Treatment
Most gum disease is painless in its early stages. Do not wait for pain before seeking assessment — these signs indicate that treatment is needed now.
| Symptom | What It Indicates | Action Required |
|---|---|---|
| Gums bleed when brushing or flossing | Gingivitis — gum inflammation requiring professional cleaning and treatment | Book a check-up this week |
| Gums look red, swollen or feel tender | Active gum inflammation — early gum disease | Book a check-up this week |
| Persistent bad breath that does not resolve with brushing | Bacterial infection in the gum pockets — a classic sign of periodontal disease | Book a periodontal assessment |
| Gums appear to be pulling away from teeth — teeth look longer | Gum recession — indicates progressing periodontitis with bone involvement | Book promptly — do not delay |
| Teeth feel loose or have shifted position | Advanced bone loss — teeth losing structural support | Book urgently — tooth loss risk |
| Gaps appearing between teeth that were previously close | Bone loss allowing teeth to drift — advanced periodontitis | Book urgently |
| Pus between teeth and gums | Active infection — abscess forming in the periodontal pocket | Book same day if possible |
| Sensitivity to hot and cold at the gum line | Exposed root surfaces due to gum recession — indicates progressing disease | Book a periodontal assessment |
Periodontal Treatments Available at Toothway Dental, Gachibowli
Scaling and Root Planing — Deep Cleaning
Scaling and root planing is the primary non-surgical treatment for periodontal disease. It involves the thorough removal of plaque, tartar and bacterial deposits from above and below the gum line, followed by smoothing of the root surfaces to discourage bacteria from reattaching.
Performed under local anaesthesia, the procedure is usually completed in two to four appointments — one or two quadrants of the mouth per session. A reassessment 6 to 8 weeks after completion evaluates the gum response and determines whether further treatment is required.
For mild to moderate periodontitis, scaling and root planing produces significant improvement in the majority of cases — often eliminating the need for surgery when combined with thorough home care.
Laser Gum Treatment
Laser periodontal therapy uses a dental laser to precisely remove infected gum tissue and bacteria from periodontal pockets. The laser’s energy targets diseased tissue while leaving healthy tissue intact — a level of selectivity that conventional instruments cannot achieve.
Compared to conventional surgery, laser treatment involves less bleeding, faster healing, reduced post-operative discomfort and in many cases requires no sutures. It can be used as a standalone treatment for appropriate cases or alongside scaling and root planing for more thorough bacterial reduction in deep pockets.
At Toothway Dental, laser gum treatment is delivered by Dr. Deepika Reddy, whose specialist training includes laser periodontal therapy.
Surgical Periodontal Treatment
When non-surgical treatment cannot fully access deep periodontal pockets or when significant bone loss has occurred, surgical intervention may be required to arrest disease progression.
Flap surgery allows the periodontist to fold back the gum tissue, directly access and clean the root surfaces and bone, then reposition the tissue for optimal healing. Bone grafting and regenerative procedures may be incorporated to rebuild bone lost to periodontitis.
Surgical periodontal treatment is performed under local anaesthesia and is well tolerated. Post-operative instructions and healing support are provided at Toothway Dental — patients typically return to normal activity within a few days.
Periodontal Plastic Surgery
Beyond treating disease, Dr. Deepika‘s expertise in periodontal plastic surgery allows Toothway to address cosmetic gum concerns including uneven gum lines, excessive gum display (gummy smile) and gum recession that has exposed the root surfaces of teeth.
Soft tissue grafting can cover exposed roots, reducing sensitivity and protecting the root from further recession. Gum contouring reshapes the gum line for a more symmetrical, proportionate smile.
Treating Gum Disease Before Dental Implants
Dental implants cannot be successfully placed in a mouth with active gum disease. The same bacteria that destroy natural teeth attack the bone around implants — a condition called peri-implantitis — which is the leading cause of implant failure.
At Toothway Dental, Dr. Deepika Reddy leads both the periodontal treatment and the implant planning. When gum disease is present in a patient seeking implants, she treats and stabilises the periodontium first — then oversees the transition to dental implants placement under the same roof, ensuring continuity of care.

How to Prevent Gum Disease — and Protect Your Treated Gums
Gum disease is largely preventable — and recurrence after treatment is controllable with consistent home care and professional monitoring.
Daily Home Care
- Brush twice daily for 2 minutes using a soft-bristled toothbrush
- Floss once daily — flossing is non-negotiable for gum health
- Use an electric toothbrush if possible — significantly more effective than manual brushing for plaque removal
- Use an antibacterial mouthwash as recommended by your dentist
- Quit smoking — smoking is the single biggest risk factor for gum disease progression and treatment failure
- Manage diabetes if applicable — uncontrolled blood sugar dramatically worsens periodontal disease
Professional Maintenance
- Attend your scheduled periodontal maintenance appointments — every 3 to 6 months if you have a history of gum disease
- Professional cleaning removes tartar that brushing cannot eliminate — regardless of how well you brush
- Inform your periodontist of any medical changes — diabetes, new medications, pregnancy all affect gum health
- Tell us immediately if gum bleeding, swelling or tenderness returns between appointments
- Regular X-rays to monitor bone levels — bone loss is invisible without imaging
- A personalised maintenance schedule will be agreed at your Toothway Dental review appointment
